SS-31 (Elamipretide)

SS-31, also designated elamipretide, MTP-131 and Bendavia, is a synthetic aromatic-cationic tetrapeptide (D-Arg-2',6'-dimethyltyrosine-Lys-Phe-NH2) from the Szeto-Schiller peptide series, characterised by its localisation to the inner mitochondrial membrane. Investigators have examined its binding to the anionic phospholipid cardiolipin, together with cytochrome c peroxidase activity and mitochondrial respiration endpoints, in isolated mitochondria and rat renal ischaemia models (Birk et al., Journal of the American Society of Nephrology 2013); a review of the series summarises this cell-based and rodent work (Szeto, British Journal of Pharmacology 2014). Human data exist: a randomised placebo-controlled crossover trial with open-label extension (TAZPOWER) examined six-minute walk distance, fatigue scores and knee extensor strength in 12 individuals with Barth syndrome (Reid Thompson et al., Genetics in Medicine 2021). In September 2025 the US FDA granted accelerated approval to an elamipretide hydrochloride pharmaceutical formulation for Barth syndrome; research-grade SS-31 is not that pharmaceutical product.
