How they differ
Different receptor families
Cagrilintide is a long-acting analogue of amylin, a hormone co-secreted with insulin, and acts at amylin and calcitonin receptors. Tirzepatide acts at the GIP and GLP-1 receptors. They are studied together in the literature precisely because the pathways are distinct.
Molecular size
Cagrilintide is the smaller molecule. Both are lipidated for extended half-life, using acylation to promote albumin binding, but the parent peptides differ substantially in length and sequence.

Cagrilintide appears frequently in combination research alongside GLP-1 agonists, which is the context in which most published work on it has been done. Tirzepatide is characterised extensively as a single agent.
